Is compounded semaglutide safe?

Compounded semaglutide is everywhere online, often at low prices, which makes people understandably wary. Here's a clear, honest answer to whether it's safe, and what actually determines that.

Reviewed for accuracy·Last updated June 2026

The short answer

Compounded semaglutide is not FDA-approved, and the FDA does not review compounded drugs for safety, effectiveness, or quality, so its safety cannot be assured in the same way as an FDA-approved medication. It is not the same as, or a generic version of, brand-name drugs. If a licensed provider prescribes it for a specific medical need, it should come from a licensed US compounding pharmacy through a certified, physician-led program. Avoid anonymous online sellers.

Key point: Safety is about WHO you get it from, not whether it's labeled 'compounded.' Compounding is a long-established, legitimate part of medicine.

What compounding actually is

Compounding is the preparation of a medication by a licensed pharmacy for an individual patient. Compounded GLP-1 medications are not FDA-approved and are not the same as brand-name drugs. Under federal compounding rules, a provider may prescribe one only for a patient-specific medical need, not because it costs less.

What makes a compounded source safe

The red flags that signal danger

Important: The FDA has acted against telehealth companies for unsafe or misleading compounded-GLP-1 marketing. That's not a reason to fear compounding itself, it's a reason to choose a certified, physician-led program and avoid anonymous sellers.
